Nurse, RN or LPN, PT Adult Day Program

This is a Part-Time Day shift role that will work Monday & Wednesday for week 1 then Monday, Tuesday & Wednesday for week 2 of the pay period. 

The Nurse, Registered or Licensed Practical, for our Open Circle Adult Day Program in Hopkins will be responsible for:

  • Providing skilled nursing care to individual participants as needed.
  • Supervising and assisting Personal Care Coordinator(s).
  • Providing medication administration & accuracy of records.
  • Planning and presenting with Program Director a 1-hour programs a month on various health/wellness topics; as well as providing individual or group counseling on good health and healthy habits.
  • Handling case management for up to 30 participants.

  • Be able to work Monday and Wednesday one week alternating with Monday, Tuesday, and Wednesday the next week. 
  • The position is about 45 hours a pay period (2 week period)

Qualifications:                   

  • Current Minnesota licensure as Nurse (RN or LPN)
  • One or more years nursing experience desirable                                         
  • Geriatric nursing experience preferred      
  • Strong communication skills                                   
  • Dependable and responsible                                         
  • Current CPR/First Aide required within 3 months of employment
