Augustana Care is a non-profit organization that provides senior housing, health care, and community based services to older generations and others in need and helps residents live the lives that most inspire them. Augustana Health Care Center of Moose Lake, an affiliate of Augustana Care, is a 72-bed long-term care facility serving Moose Lake, MN and the surrounding communities. We are currently seeking a Part Time Employee Resource Connector. 

This position is supported by a 2 year grant from Leading Age of Minnesota.

General Description

This position will assist employees to be successful in their employment by strengthening their social and emotional well-being.  This position will work to strengthen staff-team effectiveness, assist with interpersonal relationship issues and improve poor communication skills. This position will provide resources that provide options for life challenges employees may face. In order to establish rapport with staff, must be present in their work areas and have the ability to demonstrate teamwork. Must be flexible in scheduling to meet various needs of a 24/7 workforce.


Prefer experience in counseling, sociology or education may be helpful.

This position requires good listening and organizational skills, ability to help others look introspectively, knowledge on how to guide people with step-by-step instructions, and ability to follow up with staff to make sure instructions are followed and goals are met.

This position requires creative problem-solving ability.

Must demonstrate awareness of self-care and set the example for ways to “take care of yourself” during stressful times, with an emphasis on work-life balance.

Must be able to establish professional boundaries in all aspects of this position.

Essential Functions/Responsibilities

Social and Emotional Support

Provide services to employees by:

Assessing difficulties the employee may be having that are interfering with optimal employment performance.
Consulting with employee to discuss problems and difficulties and assist the employee in seeking solutions.
Provide one-to-one support to employees who are experiencing difficulties at home or work.
Consulting with supervisors as necessary.

Exploring potential areas of interest or concern regarding social issues.

Providing information to employees regarding other agencies that provide preventive education programs.

In conjunction with HR Director and Staff Development Director, identify and schedule employee educational sessions related to topics of special interest.

Partnering with other community agencies to promote and develop individual and family well-being.


Build relationships with staff at all Three Links sites as well as with community agencies.

Refer employees to appropriate agencies as necessary.

Work as a team with Human Resources, Supervisors and Management Team (as appropriate) to improve employee situations.

Establish a resource list of community agencies for personal use and to provide to employees as necessary. (Geographic area includes Moose Lake and surrounding communities.)

Maintain liaison with community agencies.

Administrative Duties

Will spend a portion of time at all programs and facilities of the organization.

Maintain files and statistics of activities.

Complete and submit standard monthly reports to the Management Team.

Attend Management Meetings.

Provide objectives and initiatives to the Management Team, which will be monitored via monthly reports. There will also be an annual review of goals and objectives as part of the performance review process.

Determine ways to interact with staff to establish professional rapport.
